Hearing loss in working musicians: why the quiet days count too

Musicians and music teachers are an occupationally noise-exposed workforce that almost no hearing conservation programme covers. Here is the actual Australian standard, the dose maths, and what protects you.

If you play or teach music for a living in Australia, you are part of an occupationally noise-exposed workforce, and almost certainly not covered by any hearing conservation programme. The national exposure standard is an eight-hour average of 85 dB(A) or a peak of 140 dB(C). Orchestral, band and teaching settings routinely exceed it. Pooled international research puts hearing loss among professional musicians somewhere between 25.7% and 38.6%, and tinnitus between 26.3% and 42.6%, against general-population rates roughly a third of that. The single most effective practical protection is a set of flat-attenuation musicians' earplugs — not foam, and the reason is not the one most people assume.

What follows is the dose maths, the actual Australian law, and an honest account of what is and is not recoverable.

Two studies, two numbers, and why you should not average them

There are two major bodies of prevalence evidence and they disagree. That disagreement is more informative than a single tidy figure.

A 2018 systematic review pooling 41 studies and 4,507 professional musicians found hearing loss in 38.6% overall — 63.5% among pop and rock musicians against 32.8% among classical players. Tinnitus came in at 26.3% across 17 studies, hyperacusis at 21.7% across eight.

A 2026 meta-analysis pooling 67 studies and more than 28,000 musicians across 21 countries found the opposite ordering: tinnitus at 42.6% against 13.2% in non-musician controls, hearing loss at 25.7% against 11.6%, hyperacusis at 37.3% against 15.3%. It found no significant difference between classical and pop players.

The larger study reports less hearing loss and more tinnitus. Part of the explanation is buried in its own methods: only around 37% of reported hearing-loss cases had been confirmed by audiometric testing. The rest were self-reported. Musicians notice tinnitus because it is loud and intrusive; they under-notice gradual high-frequency loss because music still sounds like music until quite late.

Both numbers are real. Neither is Australian — no Australia-specific epidemiology surfaced, so treat these as international figures that almost certainly apply here rather than as local data.

The dose is the thing, not the peak

Here is the mechanism that makes musicians' intuition unreliable.

Noise exposure works on an equal-energy basis. NIOSH's recommended limit is 85 dBA over eight hours with a 3 dB exchange rate — every 3 dB increase halves the permissible time. So:

Three decibels is a barely perceptible change in loudness. It halves your day.

The looser US enforceable standard uses 90 dBA with a 5 dB exchange rate, and the modelled difference over a 40-year career is stark: roughly 8% excess risk of material hearing impairment under the 85/3 approach against roughly 25% under 90/5. Same ears, different arithmetic.

This is why "it wasn't even that loud" is the wrong question. The right question is how many hours, at what level, added together across a day.

What Australian law actually says

Safe Work Australia's model Code of Practice sets the exposure standard at LAeq,8h of 85 dB(A) or LC,peak of 140 dB(C). Above that, a duty holder must reduce exposure so far as is reasonably practicable, and where hearing protection is being relied on, provide audiometric testing.

One detail worth getting right if you work in Victoria: Victoria has not adopted the harmonised Work Health and Safety laws used in New South Wales, Queensland, South Australia, the ACT, the Northern Territory and the Commonwealth. It regulates noise under its own Occupational Health and Safety Act 2004 and OHS Regulations 2017, through a state-specific Compliance Code: Noise. The numbers are identical. The legal instrument is not. Measurement and audiometric procedures across jurisdictions both point at the AS/NZS 1269 series — 1269.1 for assessing exposure, 1269.4 for the auditory testing itself.

The 3 dB exchange-rate framing above comes from NIOSH and the international acoustics literature rather than from a direct Safe Work Australia quotation. The Australian regulators give the standard; the equal-energy logic underpinning it is the same physics.

None of this reaches most working musicians, because the duties attach to employers and a freelance career has no single one.

Where the exposure actually comes from

Measured data on orchestras is unambiguous. A study of two symphony orchestras — 114 measurements across 106 hours — found brass players exposed at 86 to 98 dB LAeq, percussion producing peaks above 115 dB(C), and most musicians in the sample exceeding an 85 dB LAeq,8h exposure. String players' left ears received about 4.6 dB more exposure than their right, which on the exchange rate above is not a curiosity. It is a doubled dose on one side.

The sharper finding is about teaching. A study of 17 university music faculty across 200 sampled workdays measured eight-hour time-weighted averages of 87.5 dB(A) for brass instruction, with 69.4% of brass-teaching days over 85 dBA. Conducting came in at 82.1 dB(A), voice 81.5, strings 80.8, woodwinds 81.2, general music education 73.9. Overall, 33.5% of daily exposures and 88% of participants exceeded 85 dBA on at least one day.

Read that again with the dose clock in mind. Nobody thinks of a lesson as dangerous. But a teacher who runs six back-to-back lessons in a small hard-surfaced room, then plays a three-hour function that night, then practises at home, has stacked an exposure nobody measured and no programme covers. Teaching is close-range, daily, and cumulative — the three properties that matter most and feel least alarming.

What actually protects you

Distance and orientation first. They are free. Where you sit relative to a brass section or a kit changes your dose more than any accessory.

Flat-attenuation earplugs, not foam. The usual explanation — that musicians' plugs block more — is wrong. They typically block less: around 15 to 20 dB, evenly across the frequency range. Foam blocks roughly 25 dB at low frequencies but up to 40 dB at high ones. That uneven cut is why foam makes a band sound like it is playing through a pillow: the treble disappears, the bass does not, and the balance you rely on to play is gone.

So musicians take foam out. The best protection is the protection you keep in, and even attenuation is what makes that possible. Compliance beats theoretical maximum every time. In-ear monitors — but not automatically. This one is counterintuitive and worth knowing. A Vanderbilt study found players turned the volume up to their own habitual listening level regardless of whether they were using floor wedges or in-ears. In-ears help because they let you monitor adequately at a lower absolute level and add passive attenuation from the shell. They only help if you actually choose and hold a lower reference level. Switching to in-ears and running them at your old volume buys you very little.

Watch headphone practice too. The WHO-ITU safe listening standard recommends an adult listening mode of 80 dB over 40 hours a week, and 75 dB for children. A student practising in headphones is spending from the same weekly budget as the gig.

What is permanent and what is not

Be precise here, because the categories get blurred constantly.

Noise-induced sensorineural hearing loss is permanent. The hair cells do not regenerate. There is no treatment that restores what is gone.

Tinnitus has no cure. Management — cognitive behavioural therapy, sound or tinnitus retraining therapy, properly fitted hearing aids — reduces its impact on your life. It does not switch off the signal.

Hyperacusis is genuinely different. Its course can improve depending on the underlying cause, and graduated sound therapy often increases tolerance over six months to a year, with CBT alongside. This is the one place in this article where the news is better than expected.

Ringing after a rehearsal that settles by morning is not harmless. It is your ears telling you the dose was too high.

The gap nobody talks about

Here is the part that is genuinely unfair.

Australia's Hearing Services Program — the main subsidy for devices and services — requires a Pensioner Concession Card, a DVA Gold or eligible White card, an eligible Disability Support Pension, ADF or veteran status, or government-funded vocational rehabilitation. Occupational noise exposure is not a qualifying criterion. A 38-year-old session player with measurable loss from twenty years of work is not eligible.

That leaves workers compensation, which is the correct pathway for occupational hearing damage in principle. State schemes can cover subsidised hearing aids and a lump sum for permanent impairment above a jurisdictional threshold. But a claim is made against the employer responsible for the noisy exposure — and a freelance music career is dozens of venues, several schools, a handful of bands and a lot of cash-in-hand work over twenty years. The injury accumulates diffusely; the system assumes it happened somewhere specific. (Thresholds and scheme details vary by state, change periodically, and should be checked against your own state's current legislation rather than any summary.)

If you already log gigs, rehearsals and teaching hours for scheduling or invoicing, you have incidentally started building the exposure history an audiologist or a claim would want — which is worth knowing, given nobody else is keeping that record for you.

The practical advice is unglamorous. Get a baseline audiogram now, while you have something to compare against. Repeat it. Wear the plugs you will actually keep in. Sit further from the brass.

If you are also protecting a young player's hearing, that is a different set of thresholds and worth reading separately.

No single lesson feels loud; six back-to-back lessons, five days a week, is an occupational exposure.

Is tinnitus reversible?
No. Tinnitus has no cure, and noise-induced sensorineural hearing loss is permanent because the damaged hair cells do not regenerate. Both can be managed — CBT, sound or tinnitus retraining therapy, properly fitted hearing aids — but management reduces impact rather than removing the cause. Hyperacusis is the exception and can sometimes improve.

Can Australian musicians claim workers compensation for hearing loss?
Occupational noise-induced hearing loss and tinnitus can be pursued through state workers compensation schemes, which may cover subsidised hearing aids and a lump sum for permanent impairment above a jurisdictional threshold.
